The basic idea is you use DVD Decryptor in no file break mode so that you get a complete .VOB file. This has MPEG-2 video and PCM audio.
On a PC, you then use VIdeola to convert the MPEG-2 to H.264. Only tricky part is that if you have anamorphic video then you need to use the profile which is 320x178 rather than 320x240 pixels.
